The Elements of Artificial Intelligence, by Tanimoto deal with all aspects and give examples of LISP code used in MYCIN and the other reasoning methods. It also gives further example of fuzzy logic.
Further details on MYCIN can also be found in The Handbook of AI, Vol 2, Ch. 8. Volume 3 (Ch 11) also gives more details on belief systems.
One excellent reference on this subject is Probabilistic Reasoning in Intelligent Systems: Networks of Plausible Inference by Pearl, Morgan Kaufmann. The book gives comprehensive treatments of Bayesian networks and Dempster-Shafer calculus. In the book Pearl puts forward many arguments in favour of Bayesian networks. Examples from many AI areas are also given.
A excellent collection of research papers is "Readings in Uncertain Reasoning", Ed Shafer and Pearl, Morgan Kaufmann, 1990.
